To be honest, I think someone tried to fire it with a cleaning rod still inside the barrel.
My first company commander did this to his .50 cal M2HB. Left a sighting device in the end of the barrel, forgot about it and when he engaged his targets on the range that night… blew the barrel out like a flower head.
Round detonate in the chamber. Double-char potentially, since all the sniper ammo is hand loaded. So, some guy snoozed, put 2x as much powder as needed in the case, and when the poor shooter pulled the trigger, instead of a terrorist’s/freedom-fighter’s head exploding, his gun did. Looking at how it bowed up instead of blow up, its a testament to the good engineering in the M82.

Caused by a dirty barrel maybe? I’ve seen shotguns in shreds because of debris in the barrel.
